Begin by giving your form a title.

As you proceed, all changes will be automatically saved at each step.

A blank form will have one default question available. If you click on the dropdown where it says Short Answer you will see a list of options:

  • Short Answer

  • Long Answer

  • Multiple Choice

  • Checkboxes

  • Yes/No

You can additionally turn the question into a form element such as a Heading or use Formatted Text if you need to enter information to be read using familiar formatting tools.

You can also choose Signature to collect a digital signature on a form and Date to add the date of completion.

To add additional questions or elements to the form click Add a Question

When finished, click the arrow in the corner to exit.
