Create and Submit NCHSAA 5-Quarter Basketball Form

1. Navigate to Your Game Schedule

1. Go to Game Schedules in the left navigation menu.

2. Locate the Varsity Basketball game for which the 5-Quarter Form is required.

2. Open the Game and Access Documents

1. Click on the varsity game.

2. Select the Documents tab in the right-side panel.

3. Create the 5-Quarter Form

1. Click Create Document.

   
   
   

2. Choose NCHSAA Basketball 5 Quarter Form (2025-26) from the list.

4. Complete Page 1: School Classification & Game Location

1. Enter the Host School Classification.

2. Enter the Visiting School Classification.

3. Verify the game information (sport, competition level, contest date).

   

4. Enter the Location of Game.

5. Click Next.

5. Home Team Administrator: Declare Player Eligibility (Page 2)

1. Indicate whether you have any players who meet the 5-Quarter criteria:

   • Select No if no players need to be declared.

     

   • Select Yes to enter player information (up to seven players).

     
     
     

2. After entering player info (or selecting "No" above), click Next.

6. Home Team Administrator: Sign the Form

1. Enter your role/position.

   
   
   

2. Enter your signature.

   
   
   

3. The second signature field will be completed later by the Away Team administrator.

4. Click Next.

7. Finish the Home Team Section

If you do not need to review Away Team player information, click Done.

8. Away Team Administrator: Review and Sign

The Away Team will see the already-started form under Game Documents and can click View Report.

 

If the Home Team listed players

• The Away Team signs at the bottom of Page 2 to acknowledge the information.

  
  
  

• Click Next, then Done.

  
  
  

If the Away Team has players to declare (Page 3)

• Follow the same steps as the Home Team beginning at Step 5.

  
  
  

• If the Away Team enters player information, the Home Team administrator must acknowledge/sign before the form is considered complete.
